AND WITNESSETH that in consideration of the sum of Rupees . . . . . . . . .  (Rs . . . . . . .) only received by me from C.D., aged about . . . . . . . . . . years, son of . . . . . . ., resident of . . . . . . . . . ., (hereinafter called the ‘‘Assignee’’) receipt whereof I have separately given this day, I hereby assign and transfer under the provisions of Section 38 of the Insurance Act, 1938, ALL THAT Policy of Insurance on my life issued by the Life Insurance Corporation of India, dated the . . . . . . . and bearing the number . . . . . . . . . . in the sum of Rupees . . . . . . . . (Rs . . . . . . . . . .) only and all benefits to arise thereunder UNTO the said C.D.

AND I, the said insured, do hereby covenant with the said Assignee that I will regularly and punctually pay to the said Insurance Corporation all premia and moneys due under the said Policy and will not do or cause to be done anything whereby the said Policy is diminished in value or is rendered void or voidable or the Assignee, his heirs, administrators or representatives are prevented from realising the money or moneys due under the said Policy.
