I, the undersigned AB etc., do hereby give you notice that, pursuant to s. 38 of the Insurance Act (Act IV of 1938) and by a deed dated the ……… day of ………, and made between myself of the one part and CD of, etc., of the other part, the policy of insurance effected by me with ……… Co. on my life for the sum of Rs. …………, dated the ……… day of ………, and numbered ………, and all moneys assured by me or to become payable under the said policy, were assigned by me unto and in favour of the said CD absolutely, and I request you to acknowledge receipt of this notice and register his name as the assignee of the said policy for value. The policy is annexed hereto for your making the endorsement thereon.
