REPORTS ON COMMUNITY SURVEY

Following the decision of the Director, the intern co-ordinator and the head of the department of Community Medicine, a 14 days placement in a rural community clinic, namely Shushastho, was arranged for the intern doctors of Bangladesh Medical College Hospital.

The tour program duration was lst October, 2005 to 14th October, 2005 including the journey days. Just prior to our departure, Prof. MD Hussairi,briefed us on the tour, its aims and objectives, and what was expected of us. We left with an eager mind, and much curiosity regarding what lay ahead of us.

We focused on the followingdata

1. Antenatal checkup

2. Couple protection rate 3. Sterilization methods

3. Tenatal Checkup:

1. Antenatal checkup

The people who visited the clinic were mostly ignorant about antenatal checkups and only 50% of the younger age group was informed about it. Also we found it to be more popular among women who have some form of education. However the concept is getting popularity and many expectant mothers were observed among the patients visiting the clinic. Among these women Anemia was seen to be a common problem. Almost 75% of the women seem to be suffering from this problem. However we did not see any patient with pre-clamps a or eclampsia during our placement.

Percentage of Visits of pregnant women for antenatal checkup:

• Regular Visit: 30%

• Irregular Visit: 50%

• totally absent: 20%

Danger Signs:

The Danger signs we have encountered in patients here are

•Anemia- 70%

•Oedema- 20%

•High BP- 08%

•PV bleeding- 02%

TT vaccine:

We found many women to be ignorant about the TT vaccine, although they are beginning to learn more about it.

• Immunized Women: 50%

• Non-immunized Women: 50%

Mode of Delivery:

•Home delivery 60%

•Institutional Delivery: 40%

2. Couple Protection Rate:

About 50 families were surveyed to collect data on this. The survey was conducted in Trishal, Mymensingh. We observed that this concept was more popular with educated people and the younger age group than the elderly, mostly because of religious barrier. We found that oral pill and barrier method are equally popular. However most couples are reluctant to use sterilization techniques. We found that about 70% of the 50 couples we surveyed to be using contraceptive methods.

3. Sterilization Methods:

Very few people had much idea about sterilization methods and even less were actually found to be interested. However we tried our best in informing them and motivating a positive approach toward these methods.

Knowledge of permanent sterilization: 42%.

Conclusion:

This survey provided us with a wonderful opportunity for exposure with rural people, their ailments, their financial constraints and available treatment options. Most of these people live below poverty line and are unable to bear their treatment cost, let alone costs for investigations. Their ignorance and illiteracy was also a big challenge to overcome. It is wonderful that the government along with the NGOs are working together to bring a better life to these people.

We are all very grateful to Prof. M D Hussain and the Department of Community Medicine of our medical college for giving us this opportunity and experience.
