BETWEEN Smt A.C., aged . . . . . . . years, daughter of L.N., resident of . . . . . . . . . . . , (hereinafter referred to as the Adoptive Mother) AND Smt E.G., aged . . . . . . . years, widow of. . . . . . . . , resident of . . . . . . . . . , (hereinafter referred to as the Natural Mother).

Whereas the Adoptive Mother is a spinster who has never married, and consequently has no daughter or son’s daughter living at this time;

Whereas the Adoptive Mother is desirous of adopting a daughter;

Whereas the Natural Mother who has three daughters living has agreed to give her youngest daughter O.P. aged. . . . . . .years in adoption to the Adoptive Mother;

NOW, THEREFORE, THIS, DEED WITNESSES that before a select gathering of relations and friends at . . . . . . . . (place) at . . . . (time) on this day above-mentioned the said Natural Mother has actually given her daughter, the said O.P., in adoption to the said Adoptive Mother and the said Adoptive Mother has taken the said O.P. as her adopted daughter and that on adoption the adopted daughter has been renamed O.C.
