Notice is hereby given that the undersigned AB, who was admitted to the benefits of the partnership subsisting between CD and EF (or etc.), carrying on the business of (or as), etc., at etc., under the firm name of etc., having attained majority on the ………… day of ……..………..
(if necessary), add ………. and on the …… day of ……, having obtained knowledge of the fact that he had been admitted to the benefits of the said partnership, does hereby elect to become (or, not to become) a partner in the said partnership.
