NOTICE IS HEREBY GIVEN by me, A. B., (or for and on behalf of C. D.) fully described below pursuant to Section 80 of the Civil Procedure Code, 1908 CALLING upon the Central Government/State Government to (state the act required to be done or not done) within a period of two months next after this notice has been delivered.

The information required by the Government under Section 80 aforesaid is hereunder given:

1. Name of the prospective plaintiff (give the full name).

2. Description and place of residence (give the description and residence).

3. Cause of action (state with dates, the cause of action on which the suit will be based).

4. Relief claimed by the prospective plaintiff (as given above, also state that in default the plaintiff would claim damages in the sum of Rs . . . . . . . . . . . together with all costs).
